The Florida Class āGā Statewide Firearm License requires medical certification in certain cases under the Florida Department of Agriculture and Consumer Services (FDACS). Our clinic provides medical certification in Naples & Collier County, Florida for individuals needing provider review and completion of the required medical section in form FDACS-16008. The evaluation is to determine whether the applicant is medically fit to carry a firearm under state requirements.
